I would also suggest that you review the difference between a design patent and a utility patent. You can receive both types of patent protection on the same invention. Make sure that you are not substituting a design patent for a utility patent. A design patent protects only the ornamental appearance of an invention, not its utilitarian features.
Most design patent claims are very simple. For example: "The ornamental design for a sliding case for a digital camera, as shown and described." is a claim. You need to include patent drawings with your application. And as the expression goes, "one picture is worth a thousand words." And as always if you don't know what you are doing ask an intellectual property lawyer.
